Behavioral & Physical Health Integration Program
Children’s Crisis Treatment Center (CCTC) and Delaware Valley Community Health (DVCH), in collaboration with the Pennsylvania Department of Human Services, were awarded a 5-year grant through the Substance Abuse and Mental Health Services Administration (SAMHSA) to integrate behavioral health services within two of DVCH’s pediatric health clinics.
The program offers a continuum of bilingual prevention, treatment, and support services to children and their families at DVCH’s Maria De Los Santos and Norristown Regional Health Center.
Primary Program Goals:
- Promote full integration and collaboration in clinical practice between primary and behavioral healthcare.
- Support the improvement of integrated care models for primary care and behavioral health care, to improve the overall wellness and physical health status of children presenting with behavioral health challenges.
- Promote and offer integrated care services related to screening, diagnosis, prevention, and treatment of mental health challenges, and co-occurring physical health conditions.
